Job Summary
Plan, organize, and implement physical therapy services for PACE participants through in-person assessments, treatment programs, and home modifications. Collaborate with the interdisciplinary team to develop comprehensive care plans, document clinical services, and educate participants and families on home programs. Maintain safety standards, participate in daily team meetings, and complete required continuing education while assisting the team in understanding rehabilitation needs. Full-time Monday through Friday schedule with a $5,000 sign-on bonus and medical benefits.
Required Qualifications
- Advanced degree in Physical Therapy from an accredited school of Physical Therapy
- Current license to practice Physical Therapy in the state
- Current driver's license and proof of auto insurance
Desired Qualifications
- Bilingual and fluent in Hmong preferred
- Two (2) years of experience working on an interdisciplinary team in a hospital, nursing home or community-based setting is preferable
- 1 year of experience working with a frail or elderly population preferred
