Job Summary
Evaluate patients' functional status, mobility, and rehabilitation potential to develop individualized care plans. Provide hands-on therapy to improve strength, balance, and independence while educating patients and caregivers on safety strategies. Monitor progress, adjust treatment as needed, and support safe discharge planning through collaboration with physicians and interdisciplinary teams. Document care accurately within the EMR and supervise Physical Therapist Assistants per state guidelines. Requires current DPT or Master's degree, active PT license, 1–2 years experience, and CPR certification. Flexible scheduling with autonomy in a home health setting.
Required Qualifications
- Graduate of an accredited Physical Therapy program (DPT or Master's)
- Current PT license in the state of practice
- Minimum 1–2 years of experience
- Strong clinical assessment and critical thinking skills
- Excellent communication and patient education abilities
- Current CPR certification
- Valid driver's license
- Reliable transportation
- Auto insurance
