Job Summary
Evaluate and assess patients' functional status, mobility, strength, and rehabilitation potential to develop individualized care plans. Provide hands-on therapy to improve mobility, balance, and independence while educating patients and caregivers on safety strategies. Monitor progress, adjust treatment as needed, and support safe transitions of care through discharge planning. Collaborate with nurses, physicians, and interdisciplinary teams to ensure coordinated patient outcomes. Supervise Physical Therapist Assistants in accordance with state guidelines and document all care accurately within the EMR.
Required Qualifications
- Graduate of an accredited Physical Therapy program (DPT or Master's)
- Current PT license in the state of practice
- Minimum 1–2 years of experience
- Strong clinical assessment and critical thinking skills
- Excellent communication and patient education abilities
- Current CPR certification
- Valid driver's license
- Reliable transportation
- Auto insurance
