Evaluate patients' mobility, strength, balance, and functional abilities in homes throughout Avon, MA. Develop and implement personalized treatment plans to improve movement, function, and independence. Provide therapeutic exercises, gait training, balance training, and fall prevention strategies while assessing home environments for safety modifications. Educate patients and caregivers on treatment plans and injury prevention. Collaborate with physicians and interdisciplinary teams, then complete accurate electronic documentation.
