Job Summary
Contract Hospice Physical Therapist providing occasional home-based visits in Rapid City, SD. Conducts initial assessments and, when appropriate, develops comfort-centered therapy plans in collaboration with the hospice interdisciplinary team. Provides limited, goal-directed treatment interventions, educates patients and family caregivers on safe body mechanics, positioning, transfers, and use of assistive devices, collaborates with the hospice team to align therapy with goals of care, reassesses as needed, and maintains thorough documentation in accordance with agency policy and standards. Emphasizes quality of life, symptom relief, and support for patients and families.
Required Qualifications
- Current, unrestricted Physical Therapist (PT) license in the state of South Dakota
- Graduation from an accredited Physical Therapy program
- Minimum 1–2 years of clinical PT experience; prior hospice, home health, or geriatric experience strongly preferred
