Job Summary
Conduct physical therapy services for residents, including screening, evaluation, and development of treatment plans with short and long-term goals. Interpret findings and communicate recommendations to residents, families, and the interdisciplinary team while adhering to CMS documentation standards in NetHealth EMR. Demonstrate sound judgment in planning, implementation, and follow-up of therapy programs, ensuring compliance with company and department procedures. Perform additional duties as assigned by the Administrator or Director of Rehabilitation.
Required Qualifications
- Minimum of Bachelor of Science Degree in Physical Therapy from an accredited program
- Must have an active Physical Therapy license in state(s) of practice
- Ongoing continuing education and professional development to maintain current licensure and certification
