Evaluate, treat, and restore function for patients in a hospital-based outpatient setting by performing assessments, developing individualized treatment plans, and providing one-on-one therapeutic interventions. Monitor patient progress to adjust care as needed while educating families on recovery goals and documenting outcomes accurately.
