Evaluate, treat, and restore function by performing patient assessments, developing individualized treatment plans, and providing one-on-one therapeutic interventions. Monitor progress, adjust plans as needed, educate patients on recovery goals, and document outcomes accurately while collaborating with physicians and interdisciplinary teams. Deliver patient-centered care in accordance with hospital policies and evidence-based practices within a hospital-based outpatient setting.
