Physical Therapist

Conduct patient assessments of functional status, mobility, and pain to develop and execute individualized physical therapy plans of care. Perform interventions to restore function and relieve pain while observing and reporting patient responses to the supervising nurse or physician. Instruct patients, families, and the health care team on progress and functional limitations, then complete progress notes and submit evaluations per LHC policy. Visit patients in home settings regardless of client background, ensuring all activities comply with state laws and organizational guidelines.
