Job Summary
Conduct initial evaluations and develop individualized treatment plans for residents in short-term, long-term, and outpatient settings. Administer prescribed physical therapy treatments, including therapeutic exercises, manual therapy, and modalities, while monitoring and documenting progress to adjust plans as needed. Communicate regularly with the interdisciplinary team to coordinate care and participate in care conferences and discharge planning. Educate residents and families on proper techniques to improve mobility and ensure adherence to all state, federal regulations, and Medicare guidelines. Requires current state license, minimum one year of SNF experience, and knowledge of reimbursement guidelines.
Required Qualifications
- Current state license to practice as a Physical Therapist
Desired Qualifications
- Minimum of 1 year of experience in a skilled nursing facility or similar setting preferred
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Knowledge of Medicare guidelines and reimbursement for physical therapy services
- Experience with electronic medical records preferred
