Job Summary
Conduct initial evaluations and develop individualized treatment plans for each resident. Provide hands-on therapy including exercises, stretches, and manual techniques while monitoring and documenting progress to adjust plans as needed. Educate residents and families on home exercises and collaborate with the healthcare team to ensure comprehensive care. Stay current on latest research and adhere to all state and federal regulations. Requires a current state license, Bachelor's through Doctorate degree, and minimum two years of Skilled Nursing Facility experience. Full-time, 30 hours per week, Monday through Friday, with no weekend rotation.
Required Qualifications
- Bachelor's, Master's, or Doctorate degree in Physical Therapy from an accredited program
- Current state license to practice as a Physical Therapist
- Minimum of 2 years of experience in a skilled nursing facility or similar setting
- Strong knowledge of physical therapy techniques and treatments
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Compassionate and patient-centered approach to care
