Physical Therapist - Travel Contract

Job Summary
Provide skilled physical therapy services in patient homes throughout Springfield and surrounding communities. You will evaluate, treat, and document interventions that improve function and safety. Coordinate with interdisciplinary teams and contribute to continuity of care for post-acute and chronic patients while maintaining regulatory and documentation compliance. Travel to patient homes is required, with a productivity expectation of 30 visits per week and orientation of 5 days. Required Qualifications
- Bachelor’s, Master’s, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA
- Active Illinois Physical Therapy license
- BLS certification from AHA or ARC may be required
- Experience with Home Care Home Base EMR preferred
- Productivity expectation: 30 visits per week for registered therapists
- Orientation required: 5 days
- Ability to travel within the region as needed and provide care in patient homes
- Reliable transportation and valid driver’s license for home health travel
- Previous home health experience preferred
