Physical Therapist - Travel Contract

Job Summary
Provide in-home physical therapy to patients across Westborough and surrounding MA service areas. Responsibilities include delivering patient-centered PT care, managing care plans, utilizing OASIS documentation and homecare workflows, and coordinating with travel teams. Role involves travel to patient homes (Shrewsbury, Westborough, Northborough, Bolton, Hudson, Marlborough, Framingham) with mileage reimbursement; scheduled hours typically 8:00 AM–4:30 PM, 32 hours guaranteed with up to 40 hours based on caseload. Required Qualifications
- Active Massachusetts Physical Therapy license
- Education: Associate Degree or higher in Physical Therapy (ACCEPTED: Associate, Bachelor’s, Master’s, Doctoral)
- Home health experience
- OASIS documentation experience
- BLS certification may be required
