Physical Therapist - Travel Contract

Job Summary
Provide skilled, patient-centered home health physical therapy across a varied caseload in the Livonia/Downriver community. Collaborate with interdisciplinary teams to promote patient independence and safety in the home environment. Maintain documentation in the Home Care Home Base EMR and meet productivity expectations to support access to care. Travel within the Livonia/Downriver hub area (Wyandotte, Taylor, New Boston, Lincoln Park, Allen Park, Southgate, Westland, Wayne, Garden City, Inkster, Plymouth, Canton) as needed. Required Qualifications
- Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program
- Active Michigan Physical Therapy license or ability to obtain required state licensure prior to start
- BLS certification from AHA or ARC may be required
- Experience with Home Care Home Base EMR preferred
- Productivity expectation of approximately 30 points per week
- Valid driver's license and ability to travel within the coverage area, generally within an hour radius of hub cities listed
