Job Summary
Traveling Physical Therapist position in a Skilled Nursing Facility. Responsibilities include evaluating and treating adults with post-acute and chronic conditions to support mobility and functional restoration; work independently while coordinating with interdisciplinary teams; document care using SNF/Medicare-Medicaid systems; support patients in a 13-week assignment with guaranteed hours (36–40 per week) and a first shift (08:30–16:30). Westfield, NJ location with proximity to NYC transit and local amenities; 5x8 shift structure with a 30-minute meal break. Requires active NJ PT license and BLS (AHA/ARC) as applicable; prior travel PT experience and restorative nursing program proficiency preferred.
Required Qualifications
- Active New Jersey Physical Therapist license
- Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA
- BLS certification from American Heart Association or American Red Cross may be required
- Experience working in a skilled nursing facility or long-term care setting
- Demonstrated ability evaluating and treating adults with post-acute and chronic conditions
- Experience with documentation systems commonly used in SNFs and Medicare/Medicaid regulatory requirements
- Ability to work independently and coordinate care with interdisciplinary teams
