Physical Therapist - Travel Contract

Job Summary
Provide direct home-based physical therapy to support patient safety and independence, managing a caseload focused on functional recovery and safe discharge to community settings. Collaborate with interdisciplinary teams and case managers to deliver timely, goal-directed interventions while documenting outcomes for continuity of care. Perform home safety assessments and work independently in patient homes, utilizing the Home Care Home Base EMR for telehealth or remote documentation workflows. Maintain an active Michigan Physical Therapy license and meet productivity expectations of approximately 30 points per week. No on-call rotation required.
Required Qualifications
- Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA
- Active Michigan Physical Therapy license
- Valid driver's license

- Ability to meet productivity expectation of approximately 30 points per week
- Shift days: M-F
- Caseload geography: primary hub in Macomb with travel expected within roughly a one-hour radius depending on referral patterns
Desired Qualifications
- BLS certification from AHA or ARC
- Experience with Home Health
- Familiarity with Home Care Home Base EMR
- Prior home health caseload management experience
- Comfort performing home safety assessments and working independently in patient homes
- Experience with telehealth or remote documentation workflows
