Job Summary
Travel Physical Therapist in home health in Elizabethtown, KY. Responsible for assessment and evaluation of patient care needs related to functional status, mobility, endurance, and pain; develops treatment plans, provides interventions to improve patient outcomes, and evaluates progress to support patient independence and functional recovery in community and home settings. Requires Master’s degree in Physical Therapy, active Kentucky PT license, home health experience, and HCHB EMR proficiency; involves travel within Elizabethtown and Hardin/Meade counties with a Monday–Friday schedule and no weekend/on-call requirements; includes benefits, housing options, and travel/license reimbursement.
Required Qualifications
- Master's degree in Physical Therapy
- Active Kentucky Physical Therapy license
- Previous home health experience
- Experience using HCHB electronic medical record
- BLS certification from AHA or ARC may be required
- Professional certification required as noted by state licensing
