Job Summary
Provide rehabilitative services to older adults in skilled nursing and senior living communities by delivering evaluations, individualized treatment plans, and progress documentation. Manage daily patient volumes of 8–15 patients while maintaining an 85% productivity standard and collaborating with interdisciplinary teams to improve outcomes. Support continuity of care through documentation and treatment delivery in a setting that may require floating between nearby sites. No call or weekend coverage required. This assignment offers guaranteed 36 hours per week with comprehensive benefits, including medical coverage, 401(k) matching, weekly pay, and travel reimbursement. Located in Spring Valley, MN, the role provides access to regional medical centers and outdoor recreation.
Required Qualifications
- Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA
- Active Minnesota Physical Therapist license
- Minimum 6 months of recent experience in a skilled nursing facility (SNF) or similar post-acute setting
- Experience managing daily patient volumes consistent with SNF expectations, including productivity standards
Desired Qualifications
- BLS certification from AHA or ARC
- Experience with high daily patient volumes (approximately 8–15 patients per day)
- Familiarity with productivity standards and documentation expectations in long-term care or senior living environments
- Previous travel or contract therapy experience
