Job Summary
Travel Physical Therapist in Dublin, OH; provide home health PT evaluations, develop and implement individualized treatment plans, and deliver therapeutic services according to physician orders. Supervise and evaluate Physical Therapy Assistants and Home Health Aides. Conduct home environment evaluations, document patient progress, participate in discharge planning, and collaborate with interdisciplinary team members to ensure continuity of care. Responsibilities include initial and ongoing evaluations, developing and updating treatment plans, documenting outcomes, coordinating with physicians and the care team, and ensuring compliance with state regulations. Requires PT degree, current state license, CPR certification, reliable transportation and valid driver’s license; experience in home health and familiarity with electronic documentation/EMR. Evening shift (5x8); travel and license reimbursement may apply; comprehensive benefits and housing support may be available.
Required Qualifications
- Successful completion of an accredited Physical Therapy program (Bachelor’s, Master’s, or Doctoral degree)
- Current, unencumbered Physical Therapist license for the state of assignment
- Current CPR certification
- Must have reliable transportation and a valid driver’s license
- Familiarity with electronic documentation and standard computer functions
- Ability to supervise and evaluate Physical Therapy Assistants and Home Health Aides as assigned
