Job Summary
Travel Physical Therapist role at a Farmington, IL skilled nursing facility. Provide PT services aimed at functional mobility, fall reduction, and ADL independence; collaborate with interdisciplinary teams to optimize patient outcomes. Requires PT degree (ACCEPTED: Bachelor's, Master's, or Doctoral) and Illinois license; BLS may be required. Schedule: 08:30–16:30, five 8-hour shifts per week, onsite with potential local-area float. Required Qualifications
- Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA
- Active Illinois Physical Therapy license

- Ability to manage typical skilled nursing documentation and care planning systems preferred
