Job Summary
Deliver skilled physical therapy services in a Long Beach, NY skilled nursing facility, including assessments, individualized care plan development, and therapeutic interventions for adults and geriatric populations. Collaborate with interdisciplinary teams to support patient mobility, safety, and functional independence while documenting clinical progress and communicating with families and staff. Maintain an active New York State Physical Therapy license and BLS certification, with preferred experience in restorative nursing programs and fall prevention initiatives. Required Qualifications
- Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA
- Active New York State Physical Therapy license
Desired Qualifications
- BLS certification from AHA or ARC
- Experience working in skilled nursing or long-term care settings
- Demonstrated ability to evaluate, develop, and implement individualized treatment plans for adults and geriatric populations
- Prior travel therapy experience
- Experience with electronic medical record (EMR) systems
- Experience with restorative nursing programs and fall prevention initiatives
