Physical Therapist - Travel Contract

Job Summary
Provide skilled physical therapy in a travel home-health setting across Hope, Prescott, and Texarkana, AR. Responsibilities include evaluating adults and older adults, developing and implementing PT plans of care, and collaborating with physicians, nurses, and caregivers to achieve measurable outcomes. Maintain CMS/OASIS-compliant documentation including progress notes and 60-day summaries, participate in staff meetings and continuing education to sustain clinical competence. Schedule is evenings (2000-0400) with an 32-hour weekly guarantee; mileage reimbursed between visits; full medical benefits, 401(k) matching, 100% paid housing, travel and license reimbursement, and 24/7 recruiter support. Requires active Arkansas PT license, home-health experience, OASIS experience, CPR certification, and ability to travel within the service area. Preferred qualifications include experience with HCHB (Homecare Homebase) and supervision of PTAs or home health aides where permitted by state regulation.
Required Qualifications
- Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA
- Active Arkansas state PT license
- Home health experience
- OASIS experience
- CPR certification
- Ability to complete patient evaluations and develop PT plan of care within state-specific guidelines
- Willingness to travel within the agency service area (Hope, Prescott, Texarkana, AR)
- Shift flexibility for weekend rotation and potential holidays
