Physical Therapist - Travel Contract

Job Summary
Conduct home health physical therapy sessions to help patients recover from injuries and manage chronic conditions. Plan and execute treatment programs designed to restore physical, social, and mental functions while serving as a co-leader in an interdisciplinary approach to patient care. Collaborate with cross-functional teams to implement individualized treatment plans and document progress according to agency standards. Provide patient-centered therapy in the home setting to improve functional outcomes and support independence. Work a 5x8 day schedule with the ability to travel to patient homes within the service area, where mileage may apply.
Required Qualifications
- Doctor of Physical Therapy (DPT) degree required if license obtained on or after January 2016; or foreign equivalent.
- Active Physical Therapy license in applicable state(s) of practice required.
- Cardiopulmonary Resuscitation (CPR) or Basic Life Support (BLS or HS-BLS or RQIBLS) certification from an approved vendor required.
- Valid Driver's License and minimum age of 21 years required.
- Must be able to travel to patient homes within the service area; mileage may apply.
Desired Qualifications
- Home health experience preferred.
- Experience working in interdisciplinary teams.
- Prior travel therapy experience preferred.
