Job Summary
Travel Physical Therapist position in Mishawaka, IN providing home health services. Key duties include SOC, ROC evaluations, recertifications, and visits with documentation in HCHB. Assignment requires active Indiana PT license, home health experience, BLS, and experience with SOC/ROC processes; 27 points per week in a home health setting with full benefits, housing and travel/license reimbursement, and 24/7 recruiter support. Required Qualifications
- Bachelor's, Master’s, or Doctoral degree in Physical Therapy from an accredited program
- Active Indiana Physical Therapy license
- BLS certification from AHA or ARC
- Home health experience
- Experience with HCHB (Homecare Homebase)
- Experience performing SOC, ROC evaluations, recertifications, and revisits
