Physical Therapist - Travel Contract

Job Summary
Provide outpatient physical therapy services as a travel contract physical therapist, performing 60-minute evaluations and developing individualized treatment plans aligned with patient goals and physician direction. Deliver discipline-specific therapy, educate patients, families, students, and coworkers, and complete timely documentation including goal writing, functional quality indicators, letters of medical necessity, and charge entry in accordance with clinic policies and regulatory guidelines. Maintain licensure and adhere to state practice acts and organizational standards of conduct, while participating in performance improvement activities and contributing to a safe work environment through adherence to safety policies and safe patient handling practices.
Required Qualifications
- Bachelor’s, Master’s, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
- Active Ohio physical therapy license required to start the assignment.
- BLS certification from the American Heart Association or American Red Cross may be required.
- Experience providing outpatient physical therapy evaluations and treatment, including 60-minute initial evaluations.
