Job Summary
Coordinate care for patients recovering from injuries and managing chronic conditions within home health settings across a rural Wisconsin territory. Conduct skilled evaluations, develop individualized treatment plans, and monitor progress while collaborating with interdisciplinary teams to support safe functional outcomes. Provide services in locations including Presque Isle, Manitowish Waters, and Eagle River without on-call requirements. Work standard 8am-5pm shifts Monday through Friday.
Required Qualifications
- Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program
- Active Wisconsin physical therapy license
- At least 12 months of home health physical therapy experience within the past 3 years
- BLS certification from AHA or ARC
Desired Qualifications
- Experience with HCHB electronic medical record
- Previous travel or agency home health experience
- Experience managing a rural caseload and traveling between patient homes
