Job Summary
Provide home health physical therapy services to patients recovering from injuries or managing chronic conditions, including evaluations, treatment, and instruction on equipment use. Develop and revise plans of care, document progress notes and phone orders, and collaborate with interdisciplinary teams. Order and adapt rehabilitative equipment while applying infection control practices for all age groups with neuromuscular, neurological, and orthopedic diagnoses. Complete clinical records per agency policy and perform physical demands such as lifting and prolonged standing. Requires a current state license, CPR certification, and 1-2 years of acute care or rehabilitation experience. Previous home care experience is preferred.
Required Qualifications
- Bachelor of Science in Physical Therapy from an American Physical Therapy Association approved program, or equivalent entry-level degree in Physical Therapy
- Current State License as a Physical Therapist in the state of assignment
- Proof of current CPR certification
- Current driver's license
- Reliable transportation
- Active vehicle insurance
- 1-2 years experience as a Physical Therapist in an acute care or rehabilitation setting
- Experience with durable medical equipment
- Ability to order appropriate rehabilitative equipment
- Ability to perform physical demands of home health care including lifting, bending, pushing, pulling, prolonged standing and walking
- Ability to work a flexible schedule
- Ability to travel locally as needed
- Ability to read, write, and speak fluent English
