Job Summary
Physical Therapist – Travel Contract in Easton, MD for Home Health assignments in Caroline, Talbot, Dorchester and Queen Anne's counties. Responsibilities include performing admission assessments, therapy evaluations, discharge assessments, and routine follow-up visits; collaborate with multidisciplinary teams; travel within defined counties; evenings shift 2000-0400; 36 hours weekly; independent contractor arrangement. Requires Master’s degree in Physical Therapy, active Maryland PT license (or ability to obtain), and professional certification (BLS may be required).
Required Qualifications
- Master's degree in Physical Therapy
- Active Maryland Physical Therapist license or ability to obtain
- Professional certification required (BLS from AHA or ARC may be required)
- Experience performing home health admission assessments, therapy evaluations, discharge assessments, and routine follow-up visits
- Ability to collaborate with multidisciplinary teams
