Job Summary
Conduct home health physical therapy services for patients recovering from injuries and managing chronic conditions across rural Wisconsin communities. Coordinate care in patients' homes and collaborate with interdisciplinary teams to support safe functional outcomes. Perform skilled evaluations, develop individualized treatment plans, monitor progress, and communicate with physicians and caregivers to optimize patient safety and independence. Work an 8am-5pm schedule Monday through Friday with no on-call rotation, though travel between patient homes within the assigned territory may be required.
Required Qualifications
- Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program
- Active Wisconsin physical therapy license
- At least 12 months of home health physical therapy experience within the past 3 years
- BLS certification from AHA or ARC
Desired Qualifications
- Experience with HCHB electronic medical record
- Previous travel or agency home health experience
- Experience managing a rural caseload and traveling between patient homes
