Physical Therapist - Travel Contract

Job Summary
Evaluate patient function, develop goal-specific treatment regimens, submit written plans of care, schedule treatment frequency and duration, and educate patients and caregivers regarding condition, prognosis, and treatment rationale. Delegate responsibility to Physical Therapist Assistants and aides while ensuring all care activities follow physical therapy standards, physician orders, and facility policies. This role supports patients recovering from injuries and managing chronic conditions in a primarily outpatient setting with acute care as secondary. The position includes a 40-hour workweek guarantee, no weekend requirement, and a 50-mile radius restriction for local candidates.
Required Qualifications
- Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA
- Active Michigan Physical Therapist license as stated by LARA
- Active American Heart Association BLS certification
- Demonstrated outpatient physical therapy experience
- PT experience documented on work history and skills checklist, current to within one year
- Two supervisory reference forms completed within the last two years
- Candidates must reside outside the facility's 50-mile radius
