Job Summary
Travel Physical Therapist needed for a travel contract at a skilled nursing facility in Canton, IL. Provide evidence-based physical therapy to post-acute and skilled nursing residents to improve mobility and function, manage caseloads, and develop individualized care plans in collaboration with interdisciplinary teams. Documentation of outcomes in the medical record to support continuity of care and regulatory compliance. Shift hours are 08:30–16:30, with 5 shifts per week (35 hours guaranteed). Required Qualifications
- Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA
- Active Illinois Physical Therapist license
- BLS certification from AHA or ARC may be required
- Experience in skilled nursing or long-term care settings preferred
