Job Summary
Provide skilled rehabilitative care to residents in a skilled nursing setting, collaborating with interdisciplinary teams to restore function, improve mobility, and enhance quality of life for patients. Document patient progress and develop individualized treatment plans to support continuity of care and regulatory compliance. Manage chronic conditions while helping patients recover from injuries. Work five 8-hour shifts per week between 08:00 and 16:30, with potential flexibility to float within the client's network. Requires an active New Hampshire state license and a degree from an APTA-accredited program.
Required Qualifications
- Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA
- Active New Hampshire state physical therapy license
- BLS certification from AHA or ARC
Desired Qualifications
- Experience working in skilled nursing or long-term care settings
- Demonstrated ability to document patient progress and develop individualized treatment plans
- Prior travel therapy experience
- Experience with common skilled nursing therapy equipment and modalities
- Position may require flexibility to float within the client's network as needed
