Job Summary
Conduct skilled physical therapy in home health settings, managing patient evaluations, plan-of-care implementation, and discharge planning for recovery from injuries and chronic conditions. Supervise and evaluate Physical Therapy Assistants and Home Health Aides as assigned while maintaining electronic health records and online training platforms. Hold a current, unencumbered Virginia license and CPR/BLS certification; 2-3 years of experience preferred. Work a 5x8 day shift in Saluda, VA, with reliable transportation and valid driver's license required.
Required Qualifications
- Graduate of an accredited physical therapy program (Bachelor's, Master's, or Doctoral degree) recognized by the APTA or equivalent accrediting bodies.
- Current, unencumbered license to practice physical therapy in the state of assignment.
- Current CPR/BLS certification from AHA or ARC.
- Reliable transportation, valid driver's license, and required agency liability coverage for home visits.
- Must meet state-specific licensure and practice requirements applicable to Virginia and any other state of assignment.
Desired Qualifications
- 2-3 years of physical therapy experience preferred.
