Job Summary
Provide high-quality rehabilitative care to skilled nursing residents in Englewood, NJ as a Traveling Physical Therapist. Develop and implement individualized treatment plans to support functional recovery, falls prevention, and discharge planning; document outcomes in accordance with facility and regulatory standards; collaborate with an interdisciplinary team; experience with geriatric populations and EMR documentation; licensed to practice Physical Therapy in New Jersey; travel assignment with housing and relocation reimbursement included.
Required Qualifications
- Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited program recognized by the APTA
- Active New Jersey state Physical Therapy license
- BLS certification from AHA or ARC may be required
- Experience in skilled nursing facility (SNF) settings preferred
- Documentation and electronic medical record experience preferred
