Job Summary
Help patients recover from injuries and manage chronic conditions in a Skilled Nursing Facility in Carbondale, PA. Provide daily care during 5x8 day shifts from 08:30–16:30 over a 13-week assignment. Maintain long-term care documentation and deliver restorative programs and mobility/transfer training. Hold an active Pennsylvania Physical Therapy License and a Bachelor's, Master's, or Doctoral degree from an APTA-accredited program. Complete Level 2 background screening and maintain BLS certification. Experience in skilled nursing is preferred.
Required Qualifications
- Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA
- Active Pennsylvania Physical Therapy License
Desired Qualifications
- BLS Certification from the American Heart Association or American Red Cross
- Experience working in a skilled nursing facility
- Experience with long-term care documentation, restorative programs, and mobility/transfer training
