Job Summary
Travel contract Physical Therapist in Chicago, IL performing evaluations, developing and implementing individualized treatment plans, documenting care, supervising discipline-specific therapy services, and participating in student education and professional development; requires PT degree (Bachelor’s, Master’s, or Doctoral) and active Illinois license, BLS certification, outpatient PT experience, ability to document medical necessity and charge entries, adherence to state Practice Act and licensure requirements, and willingness to float to other clinic locations.
Required Qualifications
- Bachelor‘s, Master‘s, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA
- Active Illinois state physical therapy license required to start the assignment
- BLS certification from the American Heart Association or American Red Cross may be required
- Experience in outpatient physical therapy required
- Experience with patient evaluation, goal writing, and developing functional treatment programs
- Ability to complete timely, accurate medical record documentation, including letters of medical necessity and charge entry
- Must adhere to state Practice Act, maintain professional licensure in good standing, and meet state-mandated continuing education requirements
- Willingness to float to other clinic locations as needed
Desired Qualifications
- Experience supervising students and participating in clinical education programs
- Prior experience in performance improvement initiatives and familiarity with competency programs for direct care providers
- Demonstrated safe patient handling and materials handling practices
