Job Summary
Travel Physical Therapist — Home Health in Mayfield, KY. Provide home-based evaluations and skilled therapy interventions to patients across Graves and Calloway counties. Requires active Kentucky PT license, BLS/CPR certification, and ability to travel to patient homes. Duties include comprehensive evaluations, therapeutic interventions to improve mobility, function, and safety, and collaboration with interdisciplinary teams. Details: 2000–0400 shifts, mileage reimbursement, benefits for travelers (medical, 401k match, housing options, travel/license reimbursement), and recruiter support. Must maintain clinical competency in outpatient and home health settings and document outcomes for quality measures and patient satisfaction.
Required Qualifications
- Bachelor’s, Master’s, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA
- Active Kentucky physical therapy license required to start the assignment
- Basic Life Support (BLS/CPR) certification required
- Ability to perform patient evaluations for therapy services in home health settings
- Willingness and ability to travel within coverage areas including Graves County and Calloway County
- Minimum education requirement: Bachelor’s degree
