Job Summary
Perform Oasis assessments using HCHB, supervise PTAs, and complete admissions, recertifications, 30-day assessments, and discharges for patients recovering from injuries or managing chronic conditions in Logan and Todd. Maintain an active state license and hold a Bachelors, Masters, or Doctoral degree in Physical Therapy from an APTA-approved school. Work Mon-Fri am to 4:30 pm with no on-call or weekend requirements, earning full-time pay with mileage reimbursement between visits. This assignment is based in Russellville, western Kentucky, offering proximity to Bowling Green, Nashville, and Lake Malone State Park for outdoor recreation.
Required Qualifications
- Bachelor's, Master's or Doctoral Degree in Physical Therapy from accredited schools approved by the APTA
- Active State License
- Home health experience
- Experience with HCHB
