Physical Therapist - Travel Contract

Job Summary
Direct home-based physical therapy for patients in a traveling capacity. Provide services in patients' homes with a primary hub in Macomb, MI and travel within roughly a one-hour radius. Duties include helping patients recover from injuries and manage chronic conditions, promoting functional independence, ensuring safe discharge to community settings, and collaborating with interdisciplinary teams. Utilize Home Care Home Base EMR for documentation; telehealth or remote documentation workflows may be used. Maintain productivity around 30 points per week and ensure quality outcomes and continuity of care.
Required Qualifications
- Bachelor’s, Master’s, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA
- Active Michigan Physical Therapy license required to start the assignment
- BLS certification from AHA or ARC may be required
- Experience with Home Health preferred; candidates new to home health will be considered
- Familiarity with Home Care Home Base EMR preferred
- Ability to meet productivity expectation of approximately 30 points per week
- Valid driver’s license and reliable transportation. Mileage reimbursed at IRS standard mileage rate
