Job Summary
Travel Physical Therapist position in Fort Smith, AR providing home health physical therapy across the agency's service area. Responsibilities include evaluating patients, developing and implementing plans of care, documenting per CMS/state requirements, coordinating with physicians and interdisciplinary teams, and traveling to nearby agency locations. Requires PT degree (Bachelors/Masters/Doctorate), Arkansas license or eligibility, Home Health and OASIS experience, and BLS/CPR certification. Shift 20:00–04:00 with 32 hours/week, no on-call weekend requirement, mileage reimbursement, and 100% paid housing option. Required Qualifications
- Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program
- Active Arkansas physical therapy license or ability to obtain state licensure prior to start
- Home Health experience required
- OASIS experience required
- BLS/CPR certification required
- Ability to complete patient evaluations, develop plans of care, and document per state and CMS requirements
- Experience coordinating care with physicians and interdisciplinary teams
- Ability to travel within the Fort Smith service area and to nearby agency locations as needed
