Required Qualifications
- Active Connecticut Physical Therapist license
- Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA

- Patient evaluation and documentation experience including home safety assessments and discharge planning
- Ability to manage caseload independently and coordinate care with interdisciplinary teams
