Job Summary
Provide skilled physical therapy services to home health patients in Demopolis, AL while working in a community-based setting. Perform patient evaluations and deliver therapeutic interventions according to physician orders. Supervise and evaluate physical therapy assistants and home health aides as assigned. Maintain documentation, communicate with physicians and interdisciplinary team members, and evaluate the home environment to recommend appropriate safety or treatment modifications. Reliable transportation, valid driver’s license, and agency-required liability insurance may be required. Assignment includes evenings (5x8) with potential travel between patient homes within the service area and occasional floating to other service locations as needed. Required Qualifications
- Bachelor’s, Master’s, or Doctoral degree in Physical Therapy from an accredited program approved by relevant accrediting bodies.
- Current, unencumbered Alabama physical therapist license required prior to the start of the assignment.
- Current CPR/BLS certification from American Heart Association or American Red Cross.
- Experience providing physical therapy in home health or community-based settings preferred.
- Ability to supervise PTAs and home health aides, including evaluation and delegation of treatment plans.
- Familiarity with documentation systems and basic computer skills, including email and electronic charting.
- Two to three years of physical therapy experience preferred where noted by state-specific requirements.
