Job Summary
Travel Physical Therapist needed for outpatient clinic in South Bend, IN. Responsibilities include evaluating patients, developing and modifying treatment plans, providing discipline-specific supervision and delegation, delivering functional treatment programs to support return to community activities, and educating patients, families, students, and coworkers. Maintain professional standards, confidentiality, and compliance with organizational policies. Minimum requirements include a PT degree (Bachelor's, Master's, or Doctoral) and an active Indiana PT license; BLS may be required. Preference for outpatient experience, travel therapy experience, and involvement in student clinical education. Assignment details: shift pattern 5x8. Required Qualifications
- Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA
- Active Indiana physical therapy license required to start the assignment
- BLS certification from AHA or ARC may be required
- Experience in outpatient physical therapy settings preferred
- Demonstrated competency in comprehensive patient evaluation, treatment planning, documentation, and use of functional outcome measures
- Ability to complete timely, accurate medical record documentation, letters of medical necessity, and charge entry in compliance with regulatory and clinic requirements
- Willingness to participate in student clinical education and provide appropriate supervision and evaluations (preferred)
- Maintain required continuing education and adherence to state Practice Act and professional code of conduct
