Job Summary
Perform patient evaluations, provide therapeutic services per physician orders, and supervise PTAs and home health aides as assigned in Camden, SC home health settings. Conduct home visits to evaluate the home environment as part of the plan of care while managing chronic conditions and helping patients recover from injuries. Requires a current, unencumbered South Carolina license, BLS certification, and 2-3 years of experience with familiarity with electronic documentation. Schedule is 5x8 days.
Required Qualifications
- Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA or equivalent accrediting body
- Current, unencumbered South Carolina physical therapy license
- Current CPR (BLS) certification
- Familiarity with electronic documentation and basic computer navigation
- Reliable transportation
- Valid driver's license

- Experience in home health or community-based care
- 2-3 years of physical therapy experience
