Physical Therapist - Travel Contract

Job Summary
Provide comprehensive home health physical therapy services including admissions, evaluations, reassessments, discharges, and development of individualized plans of care, in collaboration with PTAs. Travel within Delta County and portions of Dickinson and Menominee Counties within approximately a 1-hour radius of Escanaba. Requires active MI PT license and home health experience; BLS may be required. Key responsibilities include performing admissions, evaluations, reassessments, and discharges, creating plans of care, supervising/work with PTAs, and participating in weekly case conferences and staff meetings. Shift hours are 8:00 AM–4:30 PM, Monday through Friday with a 32-hour weekly guarantee; mileage reimbursement provided between visits. Required Qualifications
- Doctoral, Master's, or Bachelor's degree in Physical Therapy from an accredited program approved by the APTA
- Active Michigan Physical Therapist license required to start the assignment
- Home health experience required
- BLS certification from AHA or ARC may be required
- Ability to complete physical therapy admissions, evaluations, reassessments, and discharges in a home health setting
- Ability to create individualized plans of care and supervise/work with a PTA
