Job Summary
Conduct home health physical therapy for patients recovering from injuries and managing chronic conditions within the Milford, MA territory. Deliver daily care in Northbridge, Upton, Uxbridge, Mendon, Millville, Blackstone, and Bellingham using Homecare Homebase electronic medical records. Meet productivity targets of 6 points per day while maintaining a valid MA license, BLS certification, and reliable transportation. Work a 5x8 or 4x10 schedule with no on-call rotation. Considered for PT/COTA pairing.
Required Qualifications
- Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program
- Active MA Physical Therapist license
- BLS certification from AHA or ARC
- Valid driver's license
- Reliable transportation for patient visits within a manageable territory
Desired Qualifications
- Home Health experience
- Hospital or SNF experience
- Experience using electronic medical records
- Familiarity with Homecare Homebase
- Ability to meet productivity target of 6 points/day (30 points/week)
