Physical Therapist - Travel Contract

Job Summary
Provide in-home physical therapy services across the agency service area, including evaluating functional status, developing and implementing individualized plans of care, coordinating with physicians, nursing, and caregivers, supervising PTAs and aides as allowed, and maintaining regular documentation such as progress notes and discharge summaries. Travel within Harrison, AR service area; mileage reimbursed between visits. Shift hours include 2000–0400 with a 32-hour weekly guarantee; weekend rotation may be required. Must hold an Arkansas PT license and be eligible to practice home health following state regulations and agency standards; continuing education to stay current with standards of care.
Required Qualifications
- Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA
- Active Arkansas State Physical Therapy license required to start the assignment
- Home Health and OASIS experience required
- CPR certification required
- Must be able to complete patient evaluations, develop PT plans of care, and document progress per state and agency policy
- Ability to visit patients across the agency service area and nearby agencies as needed
