Job Summary
Travel Physical Therapist role in Home Health based in Russellville, KY, serving Logan and Todd County areas. Responsibilities include helping patients recover from injuries and manage chronic conditions, performing OASIS assessments, admissions, recertifications, 30-day assessments, and discharges, supervising PTAs, and providing coverage within assigned geographic areas. Schedule is Monday–Friday days (approx. 8-hour days), with a weekly guarantee of 32 hours and mileage reimbursement between patient visits; 100% paid housing and relocation/license reimbursement are available. Required Qualifications
- Active Kentucky Physical Therapist license
- Home health experience
- Experience performing OASIS assessments, admissions, recertifications, 30-day assessments, and discharges
