Physical Therapist - Travel Contract

Job Summary
Provide evidence-based physical therapy to skilled nursing residents to improve mobility, function, and quality of life. Collaborate with interdisciplinary teams to develop and implement individualized care plans, managing caseloads typical of post-acute and skilled nursing populations. Document outcomes and progress in the medical record to support continuity of care and regulatory compliance. Work 5 shifts per week, 8 hours per shift (08:30-16:30), with guaranteed 35 hours weekly. Active Illinois Physical Therapist license and APTA-approved degree required; BLS certification preferred. Previous travel therapy experience in long-term care settings is preferred.
Required Qualifications
- Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA
- Active Illinois Physical Therapist license
- BLS (Basic Life Support) certification from AHA or ARC
Desired Qualifications
- Experience working in skilled nursing or long-term care settings
- Previous travel therapy experience
- Experience with electronic medical records
