Job Summary
Physical Therapist travel contract in Dousman, WI for a skilled nursing campus. Provide patient-centered rehabilitation to support functional recovery, fall-risk reduction, and discharge planning across SNF, assisted living, independent living, and memory care settings. Requires a PT degree (BS/MS/PhD) and an active Wisconsin or compact license; BLS may be required. Experience with SNF/memory care and NetHealth EMR preferred. Schedule is M–F with one weekend per month; comprehensive benefits including relocation/travel/license reimbursement and housing options when eligible. Collaborate with interdisciplinary care teams to optimize outcomes for older adults.
Required Qualifications
- Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA
- Active Wisconsin state license or compact license recognized for practice prior to the start of the assignment
- BLS certification from AHA or ARC may be required
- Experience working in skilled nursing facility, assisted living, independent living, or memory care settings
- Willingness to work M–F days and one weekend per month as part of the schedule
